| 2. | Call 435 256 0315 we will go over the contract with you as you fill it out |
| 3. | Make the deposit check out to William John Page (This Johnnie’s legal name) or fill in your or credit or debit card # with all the information needed. |
| 4. | Sign and initial contract |
| 5. | Make a copy of check and contract for your records |
| 6. | Send the contract and the deposit to: |
